This retrospective case series collected real-life observations from 131 ADPKD patients with experience in ketogenic dietary interventions (KDIs). Among them, 86% reported improved overall health, 67% noted benefits for ADPKD-related issues, and 90% observed significant weight loss. Improvements in blood pressure (64%) and slight increases in eGFR (45%) were also reported. While 66% experienced common KDI-related adverse effects and 22 noted safety concerns like hyperlipidemia, 92% found KDIs feasible. These findings suggest KDIs may be safe, beneficial, and warrant further investigation in clinical trials to confirm their impact on kidney function and cyst progression.
